Throughout the workbook, you will find sample questionnaires, answer sheets, code structures, and more, along with examples and forms to aid in constructing a survey. Also included is a Team Effectiveness Toolkit CD-ROM containing all of the survey questions in their appropriate categories, the sample Team Effectiveness questionnaires, and all of the survey support materials.

The workbook has been developed for use by internal and external resource persons/consultants who are responsible for team development/implementation activities, and also team leaders and members involved in team-based initiatives.

Robert W. Bauer has over 60 years of combined experience in the design, administration, analysis, and feedback of survey data. He is the developer of several standardized survey questionnaires, including the Human Resource Climate Survey, which has been administered to more than 250,000 organization employees. He is a co-founders of Bauer & Associates, Inc., providing survey design, processing, analysis, and feedback services to clients directly and also through a network of independent consultants. He previously worked at Rensis Likert Associates (RLA) in collaboration with Dr. Likert himself, the world-renowned behavioral scientist and author who is considered to be the originator of organization survey methodology. Mr. Bauer was President of RLA from 1976-8, and Ms. Bauer was a Senior Associate. The authors have published numerous books and articles, including most recently The Question Book Goes Interactive (HRD Press, 2002) and The Question Book, 5th Edition (HRD Press, 2003).

Sandra S. Bauer have over 60 years of combined experience in the design, administration, analysis, and feedback of survey data.
