Abstract: First developed in the 1960s by Bell Laboratories, cause and effect tree analysis looks back in time to identify potential causes of a known event. “And” and “or” logic is used to graphically show combinations of events leading to this top event. One advantage of the technique is that it can help identify multiple-event failures. It can also show design and operational errors. Three examples illustrate the application of cause and effect analysis in different situations. An online continuation of the article includes examples of symbols and structures and a nine-step procedure.
